I, insert YOUR name, hereby express my intention to participate in Forever Knight War 15 on FKFIC-L with the insert YOUR faction, and hereby grant permission to other participants to portray me as a character in their writing.

Notes:

• If you are planning to play as an independent (i.e. not with a faction), omit that bit.

• Participants must provide their real names. However, this can remain confidential if you wish.  In that case, a war name will be used on this website—on your published permission slip, in the list of participants, and in war posts.
